Entrance Qualification

The applicant should have obtained at least 80% average on their second grading grade as shown in their DepEd Form 138-A.

Requirements for Filing the Application Form

  1. Application Form. (Available at the USHS Library or the Registrar's Office.)
  2. Two 1 x 1 ID photos.
  3. Original DepEd Form 138-A (Report Card)from your school (to be shown upon submission of the application form).
  4. Photocopy of DepEd Form 138-A.
  5. Application and testing fee of Php 200.00.

Examination Requirements

  1. Test permit
  2. Current or latest Elementary School ID with photo
  3. Black ball pen (bring extra)
